Job Description

The Hospice Medical Social Worker plays a critical role within our compassionate team, focusing on improving the quality of life for patients and their families facing terminal illnesses. This position entails providing emotional support, counseling, and guidance to both patients and their loved ones during one of the most challenging times in their lives. As a key member of the interdisciplinary hospice care team, the Medical Social Worker will assess the psychosocial needs of patients and families, collaborate with healthcare professionals to create personalized care plans, and facilitate access to community resources. Furthermore, the Medical Social Worker will engage in bereavement support, addressing the emotional, financial, and social aspects of end-of-life care. The ideal candidate will possess a strong understanding of the hospice philosophy, demonstrating empathy and compassion, as well as the ability to contribute to a supportive and healing environment. Qualifications:

  • Master's Degree in Social Work
  • Must have experience as a social worker
  • Must have reliable transportation
